Supplemental Material for: Osteoid Osteoma of the Proximal Tibia Coexisting with Knee Osteoarthritis in a 68-Year-Old Man: A Case Report

<p dir="ltr">Introduction: Osteoid osteoma is a benign bone-forming tumor that typically affects children, adolescents, and young adults.

Presentation in elderly patients is rare and may be diagnostically challenging, particularly when symptoms overlap with common age-related musculoskeletal disorders such as knee osteoarthritis, lumbar radiculopathy, peripheral vascular disease, or diabetic neuropathy.</p><p dir="ltr">Case presentation: A 68-year-old man presented with a 6-month history of progressive pain localized to the proximal right leg below the knee.
The pain was predominantly nocturnal, frequently awakened him from sleep, and showed poor response to neuropathic pain medications, including duloxetine, pregabalin, and gabapentin, as well as colchicine, while nonsteroidal anti-inflammatory drugs provided only temporary relief. Physical examination and conventional X-rays of the right knee demonstrated findings consistent with knee osteoarthritis; however, focal tenderness over the proximal tibia and the characteristic nocturnal pain pattern prompted further evaluation.

Laboratory investigations, lumbar spine magnetic resonance imaging, lower-extremity electromyography/nerve conduction studies, and arterial and venous duplex ultrasonography were unremarkable. MRI of the proximal tibia demonstrated subchondral cystic changes within the tibial plateau, including a lesion with a central mineralized focus. Subsequent computed tomography revealed a well-defined nidus with surrounding reactive sclerosis, highly suggestive of osteoid osteoma.
The patient underwent surgical curettage of the proximal tibial lesion. Histopathological examination demonstrated findings compatible with osteoid osteoma, and the patient reported complete resolution of nocturnal pain after surgery. At 6-month follow-up, he remained asymptomatic with no evidence of recurrence.</p><p dir="ltr">Conclusion: Osteoid osteoma should not be excluded solely because of advanced age.
In elderly patients with persistent focal nocturnal periarticular pain, especially when symptoms are disproportionate to degenerative findings, targeted computed tomography should be considered to identify a nidus and avoid diagnostic delay.</p>
