Supplemental Material for: Prevalence, Comorbidities, and Treatment Outcomes of Patients with Acromegaly in Qatar: A Retrospective National Cohort Study

Listed in figshare

Introduction: Acromegaly is associated with increased morbidity and reduced life expectancy.

Description

Data on the epidemiology, clinical characteristics, and treatment outcomes of acromegaly in Qatar are limited.

Methods: We conducted a retrospective national cohort study of patients with acromegaly followed at Hamad Medical Corporation between 1989 and 2024. Demographic, biochemical, radiological, histopathological, treatment, and outcome data were extracted from electronic medical records.

Biochemical disease control was defined primarily as age-adjusted IGF-1 within the reference range, with GH-based criteria used as supportive information when available.

Results: Fifty-eight patients were identified in an estimated population of 3.05 million, corresponding to a diagnosed prevalence of 1.90 per 100,000 population (95% CI: 1.41–2.39). The mean age was 45.2±10.9 years, and 38 patients (65.5%) were male.

The most common comorbidities were obesity (n=49, 84.5%), diabetes (n=31, 53.4%), and hypertension (n=27, 46.6%). Tumor-size data were available for 48 patients; 41 patients (70.7% of the total cohort; 85.4% of those with available tumor-size data) had macroadenomas and 7 (12.1% of the total cohort; 14.6% of those with available data) had microadenomas. Transsphenoidal surgery was performed in 51 patients (87.9%), including 27 (52.9%) at HMC and 24 (47.1%) outside Qatar.

Radiotherapy was documented in 7 patients (12.1%). Medical therapy included octreotide in 25 patients (43.1%), cabergoline in 19 (32.8%), and pasireotide in 1 (1.8%); no patients received pegvisomant. Postoperative MRI showed no visible residual tumor in 18 patients (31.0%), residual tumor in 24 (41.4%), and MRI not performed/unavailable in 16 (27.6%).

At last follow-up, biochemical data were available for 50 patients; 19 (38.0%) had controlled disease and 31 (62.0%) had persistent active disease.

Conclusion: This study provides the first national estimate of diagnosed acromegaly prevalence in Qatar. The relatively low diagnosed prevalence, combined with a considerable burden of obesity, diabetes, hypertension, macroadenomas, and persistent active disease, likely reflects significant diagnostic delay in this population.

These findings highlight the need for earlier diagnosis, centralized multidisciplinary pituitary care, and structured long-term follow-up.
