Risk Factors for Severity and Mortality in Heat Stroke Patients: A Single-Center Retrospective Study
Listed in ScienceDB
Purpose:To investigate the risk factors for severe illness and in-hospital mortality in heat stroke (HS) patients and to evaluate the early predictive value of relevant clinical indicators.Materials and
Description
Methods
A retrospective analysis was conducted on 27 HS patients admitted to our hospital from January 2023 to January 2025. Patients were divided into a severe group (n=17) and a non-severe group (n=10) based on disease severity, and into a survivor group (n=23) and a non-survivor group (n=4) based on in-hospital outcomes. Univariate analysis, Spearman correlation analysis, and receiver operating characteristic (ROC) curves were used to assess the clinical value of each indicator.Results:(1) The severe group had significantly higher proportions of underlying comorbidities (82.4% vs. 20.0%, P=0.003), higher initial heart rate (141 vs. 90 bpm, P=0.005), higher initial body temperature (39.38℃ vs. 37.94℃, P=0.022), and higher APACHE II scores (22.24 vs. 11.10, P<0.001), but significantly lower serum calcium levels (1.10 vs. 2.04 mmol/L, P=0.025) compared with the non-severe group.

(2) Spearman correlation analysis showed that serum calcium (r=-0.708, P<0.001) and initial body temperature (r=0.675, P<0.001) had the strongest correlations with APACHE II scores. (3) The non-survivor group had significantly higher initial body temperature (40.10℃ vs. 38.63℃, P=0.037), cardiac troponin I (cTnI) levels (2.815 vs. 0.031 ng/mL, P=0.009), and APACHE II scores (31.25 vs. 16.57, P=0.007), but significantly lower platelet counts (83.50 vs. 182.50×10⁹/L, P=0.020) compared with the survivor group.
(4) ROC curve analysis showed that the areas under the curve (AUC) for APACHE II score, cTnI, and platelets in predicting in-hospital mortality were 0.957 (95% CI: 0.878–1.000), 0.913 (95% CI: 0.803–1.000), and 0.870 (95% CI: 0.707–1.000), respectively. The optimal cut-off values were ≥24 (sensitivity 100%, specificity 82.6%), ≥0.435 ng/mL (sensitivity 100%, specificity 87.0%), and ≤106×10⁹/L (sensitivity 75%, specificity 91.3%), respectively.
