Association of serum uric acid levels with pre-frailty and frailty in older adults

Objective To investigate the association between serum uric acid levels and pre-frailty and frailty among older adults.

Methods
This study conducted a cross-sectional analysis using data from the National Health and Nutrition Examination Survey (NHANES), a publicly available database in the United States. Participants were categorized based on the Frailty Index (FI) into three groups: non-frailty (FI ≤ 0.15), pre-frailty (0.15 < FI ≤ 0.25), and frailty (FI > 0.25). Weighted logistic regression, subgroup analyses, and nonlinear modeling were employed to systematically investigate both the linear and nonlinear associations between serum uric acid levels and frailty status.Results A total of 10,005 participants were included in this study.

After adjustment for covariates and using the non-frailty group as the reference, the odds ratios (ORs) of serum uric acid levels for pre-frailty and frailty were 1.146 (95% CI: 1.081–1.214) and 1.236 (95% CI: 1.157–1.320), respectively. Serum uric acid levels showed a nonlinear association with both pre-frailty and frailty. The cutoff points at which the ORs for pre-frailty and frailty became significantly greater than 1 were 6.574 mg/dL and 6.497 mg/dL, respectively.
Before and after the inflection points of 6.754 mg/dL for pre-frailty and 7.100 mg/dL for frailty, the ORs (95% CIs) of serum uric acid levels increased from 1.080 (1.009–1.156) and 1.060 (0.980–1.147) to 1.413 (1.164–1.715) and 2.234 (1.711–2.918), respectively. An interaction effect was observed between sex and the association of serum uric acid levels with frailty (P = 0.003).Conclusions Serum uric acid levels in older adults are positively associated with frailty risk, and this association shows sex differences.
Female individuals may derive greater potential benefits from intervention.
