Barrier and enablers to the implementation of capsule sponge technology in primary care

Listed in ZivaHub and Deakin Research Online and DMU Figshare — shown once because both records carry DOI 10.17034/32805686.v1

Oesophageal adenocarcinoma is a devastating diagnosis.

Description

With five-year survival less than 20%, early diagnosis is key to improving patient outcomes. However, early detection in general practice can be extremely challenging as many patients are asymptomatic until later stages of disease.

In recent years, the development of minimally invasive technologies has offered an alternative pathway to identify those individuals most at risk.

Barrett’s oesophagus, a known precursor lesion to oesophageal adenocarcinoma, can be identified by use of the novel capsule sponge technology. Research has found that the capsule sponge is better able to identify Barrett’s oesophagus than current standard practice. It is easy to perform, cheap and quick to complete, enabling it to be potentially introduced in a general practice setting.

The aim of this research is to understand the barriers and enablers for implementation of capsule sponge technology in primary care as perceived by general practitioners.

This qualitative study involved semi-structured interview with fifteen GP participants from the United Kingdom and Republic of Ireland.

Interviews were held virtually using Microsoft teams. Data analysis was conducted using Braun and Clarke’s framework for reflexive thematic analysis.

Three key themes were identified from the data regarding patient experience, referrals and management and primary care pressures. The first theme “If it’s Better for the Patients”: Patient Experience of Testing, focused on GP reactions to the practicalities of the capsule sponge test.

The second theme, “Nothing is Truly Black and White”: Challenges in Management and Referrals, centered around the potential impact the capsule test may have on referral processes both in a positive and negative aspect.

Finally, the third theme, “Always Comes Back to Money”: Primary Care Pressures, focused on the current strain on resources within primary care.

General practice is currently facing a number of challenges and GP reflected on workload issues, financial strains and time pressures.

The study is the first, to our knowledge, to consider the viewpoints and perspectives of GPs regarding capsule sponge implementation. The ongoing BEST4 trial, aims to assess the clinical effectiveness and health economics of the capsule sponge test for both the screening and surveillance of Barrett’s oesophagus.

It seeks to determine whether wider implementation of the capsule sponge test can improve early diagnosis and reduce mortality associated with oesophageal adenocarcinoma. Whilst patient voices have been considered throughout the BEST4 trial, we observed that GPs were not included within the research, and this is a significant gap. This research has the potential to aid policymakers and influence primary care pathways and guidelines.

If GPs do not support the introduction of a new test, its implementation will fail. It is imperative to consider any perceived barriers and address these prior to widespread use.

Thesis is embargoed until 31 July 2027.
