Epidemiological Characteristics Analysis of Hypertensive Disorders of Pregnancy in Beijing, 2013–2022

Listed in ScienceDB

Objective To analyze the epidemic trend and epidemiological characteristics of hypertensive disorders of pregnancy (HDP) in Beijing (2013–2022) to provide evidence for HDP prevention and treatment.

Description

Methods

Permanent Beijing residents who delivered in hospitals (2013–2022) were enrolled, with data from the Beijing Maternal and Child Health Data Warehouse. HDP was classified into four subtypes: gestational hypertension, preeclampsia-eclampsia, pregnancy with chronic hypertension, and chronic hypertension complicated with preeclampsia. Joinpoint regression was used to analyze prevalence trends (calculating APC, AAPC and 95%CI), and Logistic regression to identify HDP risk factors and adverse pregnancy outcome risks. Results A total of 1,726,444 parturients were included, with 135,894 HDP cases (prevalence 7.87%).

HDP prevalence increased from 6.14% (2013) to 11.42% (2022) (AAPC=8.50%, 95%CI: 6.98%~10.04%, P<0.05), with chronic hypertension complicated with preeclampsia growing fastest (AAPC=20.21%, 95%CI: 18.28%, 22.16%). Multivariate Logistic regression showed that the ≥45 years group had the highest risk of chronic hypertension complicated with preeclampsia (adjusted OR[95%CI]: 12.07[ 9 .34, 15 ]) vs. 25–29 years group.

Twin/multiple pregnancy (1.98[ 1 .95, 2 .01]), low education (2.49[ 2 .42,2.56]) and suburban residence (1.15[ 1 . 12 ,1. 17 ]) were associated with higher HDP risk. This subtype also had higher risks of preterm birth (7.12[ 6 .7878,7.47]), cesarean section (4.06[ 3 .85, 4 .28]), premature rupture of membranes (4.2[ 3 .68, 4 .76]), low birth weight (7.63[ 7 . 24 , 8 .04]) and neonatal asphyxia (2.86[ 2 . 17 ,37]).Conclusion HDP prevalence in Beijing increased continuously (2013–2022).

Advanced age, multiple pregnancy, low education and suburban residence are key HDP risk factors. Chronic hypertension complicated with preeclampsia had the most obvious growth and higher risks of adverse pregnancy/neonatal outcomes. Strengthening high-risk group screening and pre-pregnancy chronic disease management is necessary to reduce HDP burden.
