A UK Delphi consensus study on the definition and management of advanced cutaneous squamous cell carcinoma

<p>Advanced cutaneous squamous cell carcinoma (acSCC) encompasses locally advanced and metastatic disease; however, no standardized definition exists.
Description
Recognition and categorization, therefore, often rely on multidisciplinary judgment, which may lead to variations in diagnosis and management between centers. This study aimed to establish United Kingdom (UK) consensus on the definition and optimal management of acSCC.</p> <p>A modified Delphi methodology was followed.
Ten senior clinicians formed a steering group and identified four key topics for consensus statement development surrounding the challenges of acSCC, disease definition, assessment, and optimal management. Forty-two consensus statements were developed and distributed via an online survey to UK clinicians experienced in acSCC care. Agreement was rated on a 4-point Likert scale, with consensus predefined as ≥75% agreement.</p> <p>A total of 130 responses were received from dermatologists, oncologists, surgeons, and clinical nurse specialists, among others.

The consensus threshold was met for 39 of 42 statements (93%). A consensus definition, eight best-practice recommendations, and a treatment algorithm were developed.</p> <p>This study provides a UK-derived definition of acSCC, addressing the current lack of agreed terminology. The study outputs may help standardize disease classification, reduce practice variation, and support more consistent management of acSCC across UK services.</p> <p>Cutaneous squamous cell carcinoma (cSCC) is a common form of skin cancer.
Most cases can be successfully treated with surgery or radiotherapy, but some cases progress to advanced cSCC (acSCC). This means the cancer may have spread, come back after treatment, or become difficult to remove or treat safely. However, there is currently no single, widely accepted definition of acSCC, which can make it challenging for healthcare professionals to categorize these patients and choose the most appropriate treatment.
This study was therefore conducted to better identify how acSCC should be defined and managed in the UK. A group of ten healthcare professionals from the United Kingdom (UK) with experience in acSCC developed a survey containing statements about the definition, assessment, and management of acSCC. This survey was shared with a wider group of UK healthcare professionals involved in acSCC care to assess their level of agreement with the statements.
In total, 130 responses were received, and agreement was reached for 39 of 42 statements. The survey results were used to develop a UK consensus definition of acSCC, eight best-practice recommendations, and a treatment algorithm. These outputs are intended to help improve consistency in how acSCC is identified, treated, and managed across UK services.</p>
