Topical Vascular Organoid Therapy Promotes Microvascular Regeneration and Functional Recovery in Porcine Ischemic Cardiomyopathy
Listed in NCBI GEO
Description
Background
Ischemic heart disease (IHD) remains a leading cause of morbidity and mortality worldwide, with microvascular dysfunction playing a key role in the progression to ischemic heart failure. Current revascularization strategies restore epicardial blood flow but fail to address impaired microcirculation. To overcome this limitation, we developed a scaffold-free vascular organoid therapy.

Methods
An IHD pig model was created by occluding the left anterior descending artery. Bilayer vascular organoids (VOs) composed of human endothelial progenitor cells and mesenchymal stem cell–derived smooth muscle cells were transplanted onto the left ventricular 14 days after infarction. Animals were followed for four weeks with cardiac MRI and histological analyses. Results: VO transplantation preserved left ventricular ejection fraction, modestly improved regional function, and enhanced microvascular density and maturation.
Transplanted cells migrated into host myocardium, and angiogenesis-related genes were upregulated. Conclusions: Topically transplanted VOs promote microvascular regeneration and help preserve cardiac function after ischemic injury.
