Description
DermatoNet is a dataset of 1,719 smartphone-captured skin lesion images paired with structured clinical metadata, collected through TelessaúdeRS, a public teledermatology service operated by the Universidade Federal do Rio Grande do Sul (UFRGS) in partnership with the Rio Grande do Sul State Health Secretariat, serving primary care patients of the Brazilian public health system (SUS) in Southern Brazil. The dataset comprises consultations submitted between February 2017 and May 2019 and covers six diagnostic categories: Basal Cell Carcinoma (C44, n=448), Seborrheic Keratosis (L82, n=422), Nevus (D22, n=363), Actinic Keratosis (L57.0, n=244), Squamous Cell Carcinoma (C44, n=124), and Melanoma (C43, n=118).
Each of the 1,719 cases (one image per patient) carries a specialist-validated ICD-10 diagnosis established by board-certified teledermatologists through asynchronous clinical review, together with up to 50 structured binary clinical features (symptoms, prescribed treatments, and clinically derived flags), demographic variables (age, gender), and free-text clinical notes from both the referring primary care physician and the reviewing specialist.

Diagnostic ground truth is predominantly clinical rather than histopathological (1.1% overall biopsy-confirmation rate), reflecting the real-world teledermatology triage setting rather than a biopsy-centric surgical program. Unlike dermoscopy-based benchmarks (e.g., HAM10000, ISIC), images were captured under real-world teleconsultation conditions using smartphones, without standardized lighting or framing, providing an ecologically valid benchmark for testing model generalization.
Compared to PAD-UFES-20, DermatoNet additionally provides a full structured anamnesis and specialist free-text notes for every case (50 clinical features vs. up to 22) and a more balanced class distribution (3.8:1 vs. 16.2:1), bridging the diagnostic gap between primary and specialist care. Images are provided as standardized 224×224 pixel, 24-bit RGB JPEG/PNG files named DSOLCOD_{id}.ext; the mapping between image filenames and metadata rows is given by the img_id column of metadata.csv.
All personal identifiers — patient names, full dates of birth, exact consultation dates, health-unit and macro-region codes, and original attachment filenames (which in some cases had been assigned by the referring physician using the patient's name) — were removed prior to release, in compliance with the Brazilian General Data Protection Law (LGPD — Lei nº 13.709/2018). Patient age (in years) is retained as the sole demographic age indicator.
