Declining Mortality, Unequal Progress: Influenza- and Pneumonia-Related Deaths Among Patients With Leukemia in the United States, 1999–2024

Listed in figshare

Patients with leukemia are highly susceptible to severe respiratory infection, yet long-term population-level trends in influenza- and pneumonia-related mortality remain poorly defined.

Description

We analyzed final Multiple Cause of Death data from the Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemiologic Research (CDC WONDER) for 1999–2024. Deaths involving leukemia (ICD-10 C91–C95) and influenza or pneumonia (J09–J18) were assessed overall and by sex, race and ethnicity, age, urbanization, Census region, and state.

Age-adjusted mortality rates (AAMRs) and age-specific crude rates were calculated per 100,000 population, and Joinpoint regression estimated annual percent changes (APCs) and average annual percent changes (AAPCs). Among 54,304 deaths, the overall AAMR fell from 1.06 in 1999 to 0.56 in 2024 (AAPC, −2.59%; 95% CI, −2.83% to −2.37%; P<0.001). Mortality remained higher in males than females and rose sharply with age, peaking in adults aged ≥85 years.

Rural counties consistently had higher mortality than metropolitan counties. All four Census regions showed significant long-term declines, slowest in the South. Mortality also varied considerably across states.

Kentucky had the highest 2024 AAMR among states with estimable long-term trends, while Florida had the slowest significant decline. The burden–trend matrix identified states where relatively high current mortality coincided with slower long-term improvement. Overall, influenza- and pneumonia-related mortality among patients with leukemia declined substantially in the United States, although clear demographic and geographic disparities remained.

Continued infection prevention and closer attention to populations and regions with a high mortality burden are therefore warranted.
