Directory of physical residential facilities licensed in Illinois as Hospice Residences under the Hospice Program Licensing Act (210 ILCS 60) and 77 Ill.
Description
Adm. Code 280, Subpart D. A Hospice Residence is a separately licensed home, apartment building, or similar building providing living quarters where hospice services are delivered to facility residents.
Each Hospice Residence must be owned or operated by an entity that also holds a Comprehensive Hospice Program license; the residence license is distinct from and additional to the underlying hospice program license. State law caps each Hospice Residence at 16 resident beds and limits the number of licensed residences statewide. Each record represents one currently licensed residence and includes the residence name, physical location, primary administrative contact, license number, and license expiration date.

Buildings licensed under the Hospital Licensing Act, Nursing Home Care Act, Specialized Mental Health Rehabilitation Act of 2013, ID/DD Community Care Act, or MC/DD Act are statutorily excluded from the Hospice Residence definition. Licensure is administered by the Illinois Department of Public Health (IDPH), Division of Health Care Facilities and Programs, Hospice Program.
